The church was built in the historical centre of Trevignano and offers a splendid panorama. Built in the 16th century on the foundations of an ancient, Gothic style building and dedicated to Our Lady Assumed into Heaven, was completely restructured between 1780 and 1794 by the architect Pelucci.
The bell tower was made from one of the fortress towers and held 4 bells. There are only two there today, one of which has been completely restored to mend a crack.
There are some interesting works in the church, of which the most important is the fresco in the apse of the Dormition (or death) of the Virgin and her assumption and coronation in heaven by Pellegrino da Modena, a painter of the school of Raphael in 1517. Details of the fresco suggest that Raphael himself outlined the sketch, even perhaps painted part of it.
